Berkas pendaftar ENUMERATOR RS di masukkan dalam map WARNA MERAH, sedangkan ENUMERATOR PUSKESMAS dimasukkan dalam map WARNA HIJAU, kemudian dimasukkan dalam amplop dengan menuliskan ENUMERATOR RS/PUSKESMAS (pilih salah satu) di sudut kanan atas amplop.
